To speed up your time at our office, you can fill out your new patient paperwork ahead of time.

What to bring to your first appointment:

  • Insurance card

  • Drivers license or other photo identification

  • Referral forms

  • List of current medications (prescription and over-the-counter)

  • Any available hospital records, recent lab reports or other exam findings

 

Records Requests:

All dictation notes will be sent to referring physician or your cardiologist on file. If records need to be sent to another facility, practitioner, or hospital, please let us know.

If you would like a personal copy of your full record file there is a processing charge of $25.00.
